Disappointed in the care given by one of your RN's. No assessments made only blood pressure checks. Pt. has been suffering from a UTI for several weeks and the RN never realized the odor of the pt. due to incontinence or that the pt was hallucinating. I made the RN aware that a urine C&S was needed. He returned and sent out a lab person to retrieve the urine. The report states as assessed she has a UTI its been 3 weeks and no meds thus far.
