This health insurance company pays providers 40% of what everybody else pays for the same service, requires pre-authorization when nobody else does, doesn't pay claims when they paid for the same person for the same plan before and after that particular claim until 6 months later after an appeals process, denies co-workers on the same plan when one is covered and the other isn't.
