Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously Is this business in a safe neighborhood? Yes, it was in a safe enough neighborhood

Did you feel like this provider's office was understaffed? Not at all, they had plenty of staff members to help me whenever I needed assistance Did this provider use excessive foul language? No Did this provider ever postpone your appointment? No, they never postpone my appointment Does this provider maintain a professional appearance? Yes, he/she looked very professional

Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Was this provider's office clean? Yes everything was neat and sterile Did you leave the office feeling satisfied with your visit? Yes Did this provider rush your appointment? Not too much, but somewhat Was this provider late to your appointments? No, they were on time

Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in How would you compare this provider to others in his/her field that you have visited? There is no comparison! Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Does this provider always put the customer first? Yes, they always put customers first

No, I didn't notice an odor Does this provider adhere to the customer is always right policy? Sometimes, but not always Did you have to wait long after you arrived for your appointment? I had to wait for a little while.

Was it easy to find parking at this provider's office? Extremely easy, and close to the entrance Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Are you confident that this provider will continue working with you until a solution is reached? Yes, I know I can count on them to find a solution

Whenever I called with a question or concern, the staff made me feel stupid for calling. I avoided calling because I didn't want to feel stupid. I tried to cancel my first ever apt. at this office because I got called into work, and my parents recommended him. Well when I called to cancel they told me that if I cancel then I cannot reschedule...in other words EVER again, I can never use the doctor's services!!! I think that is crazy!!!
