After having issues with a secondary cataract I went to Fairview Eye Center, Dr. Kosmidis, to have treatment. This facility is no longer covered under my insurance. I explained this to their employees and was instructed to complete a document release form and the records would be sent to me. THAT WAS A TOTAL LIE. The only way I can receive my own personal records is to pay a fee of $40. Dr. Kosmidis has left the practice. Perhaps due to office management? I wonder.
