I called last week to request that my nephew’s provider complete a sports form. I was told that the clinic uses its own form and that the request would be sent to the provider. I followed up again on Monday, and a staff member informed me the form was on the provider’s desk awaiting completion. When I called back on Wednesday, I was suddenly told that Health Partners had no such form, that I needed to start the process over, bring in my own form, and expect a 7–10 business-day turnaround. I made an appointment for 8:20am and when I got there I was informed that the provider called in. I did not receive a call or my chart message telling me that, so I showed up. They then told me I could come back at 10am to see a different provider and gave me that providers name and said “you will really like her!” Then on the way home I got a call saying there actually aren’t any times for that day for me to go in. Busy location. Plenty of free lot parking & Handicap. Including entrance, bathrooms, elevator. Well rounded staff, professional, kind, seem to enjoy their positions.
