Health Care Connectors (HCC) Have absolutely been wonderful with my mother, Jacqueline Preiss over at EdgeHill. I must praise all that I communicated especially Neelam for OT, Tashura, the visiting nurse and Bhakti for PT. 👏👏 👏 We were able to get palliative care for a month which really helped mom succeed back to eating and communicating a little bit better. Thank you to the excellent care and communications that Health Care Connectors provides! From the initial call, to OT and PT intake, to the weekly calls from Lindsay to see how things are going! HCC services were arranged following my mother’s discharge from a skilled nursing facility, facilitating a smooth and reassuring transition back into the comfort of her home. Excellent comprehensive care, Especially the PT with Ashley and JJ. Ashley helped identify the muscle repair needed and gave us simple and worthwhile exercises we could do on our own. JJ brought us to a new level of acceptance and motivation to improve our health situation. It’s made all the difference. With renewed hope we face my husband’s exercises with a positive attitude and we can see the improvement.

The pluses are numerous… from the speed with which HCC dispatched their team to support my mother post stroke to the quality of their staff- we are very grateful to have this valuable resource nearby! Debbie was terrific in providing initial assessment and securing the right profiles to work with us. JJ, ( PT) was exceptional in motivating and getting through to mum on what she’d need to achieve.
