Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Does this provider treat their staff well? Yes, they treat their staff with respect

Was it easy to make an appointment with this provider? I waited longer than I wanted to, but it wasn't terrible Did they follow up with you after your appointment? No, but they said they were going to! Was this provider's office clean? No, I didn't feel comfortable at all

Does this provider maintain a professional appearance? Yes, he/she looked very professional Does this provider offer flexible appointment times? Yes Did this provider prominently display their diplomas or business license? Yes Were the waiting room chairs comfortable at this provider's office? They were average Did you spend a lot of time in the waiting room at this provider's office? The wait wasn't terrible, but it wasn't the best

Does this provider promptly return your phone calls? Not really. I usually have to wait a long time Did you feel like this provider's office was understaffed? No, they were fully staffed Are you confident that this provider will continue working with you until a solution is reached? Not really, sometimes I worry they're going to stop trying to help me Was this provider argumentative or easily angered? No, they were very calm and understanding

Did this neurologist answer all of your questions? No, I left confused and frustrated Was this neurologist friendly? Polite Enough Was there annoying music playing while you waited or during your visit? No, there was no annoying music Was the waiting area spacious? No, it was rather cramped Did this provider have an entertaining selection of waiting room magazines? No, there weren't many to choose from

Did this provider ever take personal phone calls while in the appointment room with you? Yes! He/she took a personal call right in front of me. I found it to be incredibly rude. Does this provider treat their staff well? Yes, they treat their staff with respect Did you leave the office feeling satisfied with your visit? No, I felt confused and uneasy when I left
