This dentist is just pitiful. They just up and closed no warning. I can’t even get access to my dental records which I a paid for. How do I report this?

So I took off work as I usually need to, to go to my cleaning appointment. I should have known something was off when they didn’t call to remind me of my appointment. I tried to call them the day before, no answer or way to leave a message. Arrived at my 9 am appointment, office locked up, snow not shoveled so it was impossible to park. Used to love this dentist. He took the practice over from Jeff and Jean. In the last year it has gone downhill. It started when no one called with reminders for appointments. Then they would reschedule the appointments and cancel them at 9pm the night before. Now you can't even get anyone to answer the phones or return a voicemail. I have spent 2 weeks trying to cancel my appointment and no one would answer. I even called their Maineville office and no one answered there either.
