His office should get zero stars. Been trying to get an appointment for a month. Woman that works in the office doesn't return calls and though out insurance info. I had to give it to her a second time. And again she didn't call back with if they take the insurance. I had to call her yet again just to be told we can't even find your insurance, good luck. Mind you its Horizon.
