Met with doctor and paid for visit. I was approved but state of Illinois never received certificate. Green Mind Physicians will not respond or help clear up matter. Buyer Beware
Don’t use this company, I had an appointment at 12:05 pm. No call. I had to reach out multiple times before they called me around 5pm. Then my card was denied due to the dr not filling out/answering questions. Waste of money, go somewhere else. There’s also no phone number to contact anyone, email only,
