Grand Welcome's terms and conditions, https://www.grandwelcome.com/terms-and-conditions, state "For a $75 fee or 5% of the Total Charges whichever is greater, you may cancel Your reservation by giving Us written notice at least 60 days (90 days for Hawaii Properties) before Your Check-In Date." 5% of your total charges were greater than the $75, resulting in the cancellation charge of $251.88.

Hi Kristen, We sincerely apologize for the issues at the beginning of your stay. I do see that our time has paid $407 for 1 night in a hotel room due to the oversight by our cleaning crew. This was a huge oversight from our team and we have brought this up to our local manager and the operations manager to make sure this does not happen again. Unfortunately, your refund request for 1 night was denied because of the $407 nightly rate from the hotel. If we were to refund you for the night not spent, our team would not be able to pay for the hotel and therefore, we would have had to charge you for your hotel stay.
