The Doctor is charging $150 for medical forms to be filled out including government medical forms. Dr. Opam and his staff are only interested in financial gain.

This place is a mess. For starters I was referred here by a PCP for physical therapy after surviving a car accident. I've been to this place a total of 5 times and haven't gone back since March because the service is lacking so much. My first appointment I was literally left waiting in the doctor's office for 40 mins because he "didn't know he had a patient" and in Dr Opam's words "every patient is different and needs different amounts of time.
