I had an initial evaluation at Forme in Wauwatosa. I was assured there was no charge for that evaluation. I am a veteran of these vein procedures and every place I have ever been in18 years never charged me for an initial evaluation. I told them all I wanted was spider vein treatment. I have had everything else you can remove, removed long ago. So, I thought Froedtert, Ok I will try them. I am sorry I did, because they then billed my insurance.
