Dr. Forman has been both my GP and friend for over 45 years. I have been his patient since he started his practice. He is a fine physician who treats his patients with care , compassion and a vault of expertise. He has diagnosed problems I have encountered over the years that even the so called specialists were baffled by. I trust him with my health totally and he has served my entire family extremely well. I dread the thought of ever having to replace him.

would not refer me to a neurologist for persistant headaches. i had a grand mal seizure and was taken to the er and saw the neurologist on call. i had to follow up with the neurologist and dr. forman got mad because he was not notified of it. it turned to be a tumor and it required surgery. he got nasty to me from there on.

No, they are really great about answering the phone right away Was there annoying music playing while you waited or during your visit? No, there was no annoying music Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed

Dr, Forman has been our physician for over 15 years. When he retires we have no idea what we will do for a doctor. He is great.
