Beautiful clinic. Staff are always welcoming and work hard at making you feel comfortable. Dana has treated my leg veins for years and does the BEST job in eliminating existing veins, both big and small and keeping them at bay. I have had lived in a few cities and had my veins treated and no one has done as thorough a job as Dana. She also does a fabulous job with Botox and filler treatments. Denise is the best- wouldn’t go anywhere else for Botox and filler! When I came in for my appointment I asked about Latisse and the staff got me in right away with Dr. Reitmeier, he was very helpful and friendly!
Always super welcoming with all the extra little touches that make your appointment more special! Dana is the best for Botox, her work is incredible and you always look natural.
I know I'm in great hands with Denise at Form. With years of experience and a natural approach, I trust her with my skincare needs. The staff at the front desk are always lovely to deal with, and they make amazing lattés. :)
