Forest DPC is a huge blessing to our family. The patient provider relationship is phenomenal and the care is top tier. We are sincerely grateful to be patients of Dr.Ponke, he and all the staff are incredibly intentional in all their interactions with us as patients. I took a while to read all the wonderful reviews for FDPC written about the practice and Dr. Farrago. All of the comments and good remarks still hold true for the practice. BUT Dr. Dan Swartz purchased the practice from Dr. Farrago about 5 years ago and if possible, the practice has improved with Dr. Dan. While the medical model (Direct Primary Care) is the same my wife and I like Dr. Dan better. It's about 6 years we have been in the practice and we love it.
