The TL;DR - Avoid Foothills Smiles and Orthodontics and Dr. Jarvis at all costs! On May 2, 2019, my wife and I would definitely NOT recommend this office and Dr. Jarvis. My wife went for a routine exam, was told she needed both a filling and a crown on separate teeth. The filling went OK, but then they upsold her on a "better" crown with a 5 year warranty.
