Not even worthy of 1 star. They dont cover anything that they promise you. I wrote the president of Administrative Concepts Inc., which is a part of Evolve , Steve Urban, is a non caring hole. He never even bothered to write back, because he knows his work is a scam
This company is a scam. A total joke. I spent 4 hrs w an agent to make sure I had the right coverage. I walked away assured I had the best ppo out there w minimal out of pocket. So after a yr w evolve I get a call from my dr ofc saying I can't c my dr until I pay my past due bill. What bill. Turns out not a single claim was covered. Instead of this amazing wonderful ppo where I had virtually no out of pocket and could see any dr anytime was really I get 2 visits per yr. 2. Thats it. A visit can b a routine dr.
