omg i hate that place
It's a shame how this facility left people scrounging for a new PCP when they never notified people that they were going to close.
I have been going here for 3 years and until last year this place went downhill they never answer the phone they forgot to send my meds to my pharmacy and here I am sitting here without my meds because of that …. I do not recommend . I will be looking for another place to go to .I forgot to mention I assume they changed their number because now when I call it says the number is not available
