1 star for NOT offering a crown that HMO insurance will cover. According to my Blue of CA HMO insurance, I should only pay a copay of $350 for procedure code of D2740 (all porcelain ceramic crown). I was quoted $1205.00 copay instead of a $350 copay and they will NOT honor the contractual provider rate of $350 copay because they only offer this Cerecfired crown made in house.
