Didnt have my records available, because I had to reschedule my appointment. So she was going off memory. Aren't all records electronically available. My records from another clinic should have been easily accessible, and in my chart. She got the surgeons name wrong and surgery dates. Why didnt she review my records before my appointment. Instead she went off memory from over a week before my last rescheduled appointment. I find that to be an issue, because she could have my treatment confused with another patient.
