Proceed with extreme caution. I booked an initial appointment here but chose not to move forward after that and never completed or submitted their intake forms. Despite never submitting the forms, this clinic managed to harvest my credit card information from my unsubmitted, unencrypted draft and charged me a "no-show" fee. Taking financial data from an unsubmitted web form without consent is a massive data privacy violation and a breach of PCI compliance.
