Drs. Luckhardt and Maze

Dr. David Maze was born in Elgin, Illinois. He attended Streamwood High School. He earned a Bachelor’s degree Cum Laude from Benedictine University (formerly IBC). He received an OD degree from Illinois College of Optometry in 2003. He completed a Vision Therapy and Rehabilitation residency program at the State University of New York State Optometric Center in Manhattan. He is a member of the American Optometric Association, Illinois Optometric Assn, Cornea and Contact Lens Section of the American Optometric Assn, the Neuro-Optometric Rehabilitation Association and the Optometric Extension Program.
