This "doctor" had me committed despite having literally never spoken to me. Her nurse did an intake survey, and the first thing she said to me was that I should go into a psych ward. Thankfully another doctor sprung me out as soon as I saw him, but to have decided something like that without even speaking to a patient!? Absolutely unforgivable. She ought to lose her licence.
