Doctor Wind billed my preventative checkup under the wrong code and was denied by insurance company. Since I moved to another city and they couldn't contact me, I was actually put on collection service for this! I had pretty comprehensive insurance coverage at the time so it would've been a $20 co-pay, but because they didn't bill it correctly, or contacted me in time.
