I can't believe this - Dr. Kelly closed his doors with no notice, and with absolutely no referral. When I asked for my records, they said fill out this form and send it to a processing office. I did back in August, but my new eye doctor said they never received the records. Dr. Kelly handled his closure in a totally unprofessional manner, leaving all of his patients hanging, and is inexcusable.
