I can't believe this - Dr. Kelly closed his doors with little or no notice, and with absolutely no referral. When I asked for my records, they said fill out this form and send it to this processing office, which I did back in August. Now my new eye doctor said they never received the records, and that I need to fill in this same form again. Dr. Kelly handled his closure in a totally unprofessional manner, leaving all of his patients hanging.
