Beware! This doctor works at OSF hospital @ Bloomington, IL but through his independent company and will bill your insurance company separately. And he is NOT in network so insurance company will not pay for his service which eventually will become your responsibility. In our case insurance company said provider to resubmit claim with correct billing code but this fellow instead choose to send our bill to collection agency! What a shame! No way to contact his company.
