Dr.Jayakumar made concerning billing errors. My insurance had rightfully denied his claim for services that were not provided during my visit. He then added an amendment to my visit summary that contained false information about treatments and issues not discussed during my visit so that insurance would approve this claim. The office staff let me know that they would correct this after I brought this to their attention and I had threatened to contact my insurance fraud department and local regulatory agencies.
