She broke patient confidence. She forwarded personal information to someone who was stalking me and my family. I explicitly asked her to hold certain details in confidence and she deliberately passed those details on to a former family member who was at the time and continues to threaten me, my spouse, and my children.

She abandoned care. In addition to not following up via phone in a timely fashion she left a voice mail confirming our appointment and someone else was in her office during our scheduled time. I asked her to call me and she never followed up. This is called patient abandonment.
