Ever since Jill left this practice has really went down hill. I had to request a form be filled out 5 times and got an attitude stating they filled it out but they couldn't provide a copy of the form. Office is dirty and not kept up. Nurse sits on her phone or doing her nails as you are sitting in the room for am extra amount of time
