Dr. Leventry has passed away. I needed a dentist that was an accepted provider for Dentemax Ins. When I called the number provided on 1-9-2013, the receptionist picked up and was friendly. She told me that Dr. Leventry had passed away. His replacement, Dr. Liam (I think is the name), was not an approved dentist.
