Had discussed bp med with her and latter decided to go ahead and take med but she hadn't documented previuos discussion in chart. Also called for refill and she refilled least important one and withheld asthma inhaler telling pharmacy to have me come in to see her. Same clinic Doc on call okayed inhaler. Bad documenation and really stupid withholding an asthma Inhaler.
