I moved here from out of state. I immediately got scheduled with Eric Seymour to continue my medications for my chronic skin condition. I wanted to make sure I had a local doc to continue my scripts without delay. They sent out paperwork, which I filled out and sent back, only to have to do it again the day of my appontment.
