Did you have to wait long after you arrived for your appointment? Yes, I had to wait quite a while Did this provider ever postpone your appointment? No, they never postpone my appointment Does this provider remember you and your circumstances at every appointment? Absolutely not! I constantly have to repeat and re-explain my situation!

Are you going to visit this provider again? Not if my life depended on it Was this provider's office too cold? Not at all, it was the perfect temperature Is this provider's business conveniently located? Sort of, but its pretty far away from everything Did you have to wait long after you arrived for your appointment? No, they were ready for me right after I arrived.

Were you able to get answers to simple questions from this provider without scheduling an appointment? Absolutely! I'm always able to get my questions answered over the phone right away! Did this provider seem up-to-date with the current advancements in their field? Yes, they were informing me of new advancements that were extremely recent Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! I was never uncomfortable

Would you refer this provider to a family or friend? Yes Did this provider seem up-to-date with the current advancements in their field? Yes Was this provider's office easy to locate? Fairly Easy Does this provider leave you on hold for a long time when you call? Never! I don't think I've ever been put on hold here for a long time

Was it easy to find parking at this provider's office? Yes, it was convenient Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Would you refer this provider to a family or friend? Maybe, I'm not sure if I loved them that much.

No, they were able to provide everything they said they would Did you spend a lot of time in the waiting room at this provider's office? No, my appointment started at the scheduled time

Would you refer this provider to a family or friend? No, If I had known how terrible they were I never would have gone. Did this provider listen to your input and concerns? Definitely not! I was constantly interrupted or cut off! Did this provider use excessive foul language? Definitely! I was shocked and horrified!

Did this provider rush your appointment? No Were the restrooms well-stocked at this provider's office? Yes, they had everything you could possibly need! Did you notice a foul odor when you arrived at this provider's office? Not at all, their office smelled fantastic!
