Was this provider's staff friendly? Friendly Enough Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Does this provider adhere to the customer is always right policy? Yes, they listen to all complaints and make every effort to correct the situation. Did this provider have an entertaining selection of waiting room magazines? No, there weren't many to choose from Would you refer this provider to a family or friend? Yes

Did this provider seem well-trained and experienced? Yes, they seemed very qualified Is this business handicap-accessible? Yes, it was handicap-accessible Did you have to wait long after you arrived for your appointment? I had to wait for a little while. Did this provider ever dismiss your concerns as unimportant? No, they take every concern of mine seriously

Does this provider give unbiased advice? Yes Did this provider's staff seem to respect him/her? Yes, I could tell the staff respected him/her Did you notice a foul odor when you arrived at this provider's office? Not at all, their office smelled fantastic! Did this provider prominently display their diplomas or business license? Definitely! I saw it as soon as I walked in! Does this provider remember you by name? Every single time, even if it's been months since my last appointment!!
