BEAWARE OF FALSE BILLING!! This doctor not only wrote me a referral using current work comp insurance but used an employer from 2014. I also noticed in the patient portal that this guy or the incompetent staff are using data from an old work comp injury from 2014 and is trying to bill not only them but also older personal insurance that I haven't had since 2014. I have more data than attached. I have filed complaints with the state and federal regulators do not trust this office or doctor
