Medical billing fraud. I was told that I had to pay a $80 office visit fee (I knew my insurance had $30 co-pay amount), they claimed that I didn't. Then they charged the patient after me $70 because she wasn't a first timer! Of course I received an explanation of benefits weeks later, it shows that the doctor charged me $30 co-pay (which they denied at the time of visit) and I'm still responsible for $30 (insurance did not cover).
