Does this provider maintain a professional appearance? Absolutely Not! He/She always looks like a slob. Does this provider maintain a professional appearance? Absolutely! You can tell that he/she really makes an effort to look professional

Did this provider rush your appointment? Yes, I felt like an annoyance! Did this provider ever postpone your appointment? All the time, and they usually don't notify me until right before my scheduled appointment Was this provider's office too cold? Way too cold, I thought I would catch hypothermia Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor Were the restrooms clean at this provider's office? Yes, I could tell they were cleaned regularly

Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Is this business in a safe neighborhood? Absolutely, It is the safest neighborhood around Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in Did you feel safe in this provider's care? Yes Was the waiting area spacious? Yes, extremely spacious

No Was it easy to make an appointment with this provider? Yes! It was effortless! Does this provider remember you and your circumstances at every appointment? Yes Does this provider give unbiased advice? Absolutely! I always get sound, helpful, and completely unbiased advice!

Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Did this provider answer all of your questions? Every question I had was answered thoroughly Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable.

Were the staff members able to answer your questions? Yes, they were able to answer my questions Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Did you feel like this provider's office was understaffed? Not at all, they had plenty of staff members to help me whenever I needed assistance Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for Was the waiting area spacious? No, but I can always find a seat

No, they never postpone my appointment Was it easy to find parking at this provider's office? Yes, it was convenient Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. Does this provider promptly return your phone calls? Definitely! They always call me back very quickly!
