I was sent an e-mail with 26 pages of paperwork. One form wanted to keep my credit card on file and any charges not paid by insurance or me will be charged within 30 days. I did not feel comfortable leaving a credit card with all the fraud and hacking going on so I called the office to ask if that form could be waived and if not I would cancel.
