I was a client for 14 yrs. On Aug 2019 I received a notice from the IRS regarding a premature Roth IRA distribution I took in 2017. I recall showing Dennis the 1099R during our appointment and bringing it to his attention. The money was to pay my son's tuition. Dennis failed to report the additional (tax free) income.
