We asked for an appointment because they could get us in the next week. They sent over new patient paperwork by email the next day. I filled everything out, and then on the last page I opened, I saw a release form for my personal medical history information to an "Apostolic Network of Global Awakening." The form was prefilled with someone else's name and information, meaning whoever sent out these packets is not checking to make sure they aren't releasing your personal data. I am Very displeased with the services here. I was seeing Jen Guise and she was unprofessional. She rarely called me by first name (I assume she didn't remember), and she repeatedly mistitled people in my life that I talked about during sessions, meaning she could not be bothered to remember (i.e. called my boyfriend my husband and my grandmother my mother). Chad is a very good therapist and this place is amazing. It has really helped me in the past year or so. I went there after being released from the mental hospital, I was sent to for trying to kill myself, and now I am loving life and I am so grateful, thank you so much! I see how beautiful life is now. And if you do go here and have Chad as your therapist I can guarantee he will do everything he can to help you because he genuinely cares and he is incredibly wise.
