I find it ironic other reviews on this provider focus on financial aspects because I had a similar experience! His name was given to us from United Healthcare. When we asked him for psychological testing (which is covered by insurance at 100 percent), he told us that for these types of evaluation he only accepts cash, and he wants $1500 up front!
