RESPONSE TO MANAGER: I was not unkind. When I came in, they asked me to fill out a form that I had filled out before and I was irritated by that. But I didn't say anything mean to them. I filled out the form and returned it. When the other person told me that there was a problem with insurance, she apologized and I told her she had no need to apologize, this wasn't her fault.
