Wish I could give 0 stars. Had an appointment booked and was not only abruptly cancelled - but not offered to reschedule until 4 days later. Front desk reported that the provider could not see me because of forms never sent to me, and then could not stay late because it's a "holiday weekend". Extremely unprofessional and may I say unethical in this line of work.
