This doctor doesn't deserve any stars. My 19 y/o son had multiple fractures due to a MVA. First red flag about this office and this doctor’s practice was that we were expected to complete a patient assessment form for the doctor?? Why would we be the one (my son at that matter) to complete the assessment form if the doctor should be the one performing the examination with the patient?
