I did not sign the HIPPA form. One of the office staff let me know and I stated “I do not sign HIPPA forms”, she said “OK”. The office manager later said I needed to sign the form and I said that I understood it was my right to not sign it. She then said “then I will need to cancel your appointment &you will need to leave”.
