We had to have a clearance letter sent to our primary cardiologist and we gave the office the contact information before we left, but not the fax. I called the next day on my husband behalf to give the fax number. Fax was sent and I had to follow back up a couple days later as my doctor needed the form re sent. That's when everything hit the fan they were very rude they called me a liar as I asked to just have the form sent to the nurse via email.
