My husband was a patient in the Berryville, Va location during the times between his chemo treatments for brain stem cancer at UVA in Charlottesville. The letter states that my husband's personal data, medical history, health insurance information and treatment records were accessed by a Clinical Psychologist from Chapman Senior Care without any authorization from me or my husband's physician.
