Doctor and staff get a 5 star review. The check- in a 0. I am 62 years old and have never seen anything that ridiculous, I get checked in faster in the hospital. There is no reason to ask all that stuff every time you go to the doctor, and if you are going to computerized every thing put it all in one document, no freaking reason to have so many different ones.
