The doctor office takes over 3 weeks to request a single prior authorization form from insurance, and I found it from the official insurance website... Now, I am still waiting for office's response... Billing office is the only call I received from this center.... Good luck to everyone that has to visit this health center... Now I understand why they claimed CarePoint is "Non-profit" health center.!
